Liv it Up is an organization created to honor the wishes of Olivia “Liv” Stoy. Olivia believes we should all “Liv it Up….because life is short!”
Liv It Up provides financial support to families that have a child going through cancer treatments or treatments for other life threatening illnesses. We hope families have the opportunity for special moments or experiences that they can enjoy with each other, forget about their disease and feel some family normalcy! This could include, but is not limited to, travel expenses, dinner, and hotel stays to provide family normalcy.
Eligibility Criteria
Liv It Up is open to all families with children who have cancer or other life threatening illness in Indiana or the surrounding states. Indiana will remain at the top of our fulfillment list, if funds are limited.
Families must explain how they plan to use the money to provide family normalcy.
Eligible patients must have been diagnosed when they were 18 years old and under. The patient must currently be receiving treatment.
Families are eligible to apply multiple times, up to a total maximum of $5,000.
The application must include an original letter from the primary physician or case/social worker on hospital letterhead. The letter should verify the diagnosis and treatment of the patient and be signed by the primary physician.
Applications will be reviewed and approved by the Liv it Up board on a monthly basis.
Families give Liv It Up the right to use your child’s name and likeness on our website, Facebook page and other media and promotional materials.
Families participating in the Liv It Up financially supported activity agree to forever release Liv It Up and PLMC against any and all liability, damages and claims of any kind that may be connected with or result from participation, notwithstanding any liability, damage or claim that may arise due to the negligence of Liv It Up.
Approved applicants will receive a check payable to the parent/guardian and mailed to the address on the application.
